Job summary

Pathways Healthcare in Stoneham, MA, is seeking an experienced Nurse Manager to lead the Center’s clinical team and ensure delivery of high-quality in-home care. This role requires an active MA RN/PT/OT license, BLS CPR, and strong communication along with supervisory skills.

You will oversee admissions, staffing, visits, and care plans; coordinate with physicians; support quality and regulatory initiatives; and foster a positive patient and employee experience in a collaborative environment.

Qualifications

• Active RN/PT/OT license in MA.
• Current BLS CPR certification.
• Minimum 2 years direct clinical experience; home health preferred.
• Prior supervisory experience preferred.
• Excellent written and verbal communication; computer/tablet/smartphone proficiency.

Responsibilities

• Lead and support the Center’s clinical team with day-to-day guidance and problem-solving.
• Oversee daily clinical operations including admissions, patient assignments, visits, staffing, and continuity of care.
• Lead daily clinical meetings in person once per week and virtually the other days.
• Serve as a clinical resource for staff, assisting with complex patient situations and care planning.
• Monitor care and documentation to promote quality, compliance, timely billing and outcomes.
• Develop team through feedback, joint visits, case reviews, and ongoing education.
• Partner with Recruiting and leadership to identify staffing needs and onboard clinicians.
• Monitor clinician schedules, productivity, and caseloads while balancing patient needs.

Position Details:

• Main Office location: Stoneham, MA
• Service Area: Stoneham Center Territory
• Job Type: Full Time
• Schedule: Monday - Friday
• Salary Range: $100,000.00 - $115,000.00

Responsibilities:

• Lead and support the Center’s clinical team, providing day-to-day guidance, coaching, and problem-solving to help clinicians deliver exceptional patient care.
• Oversee daily clinical operations, including new admissions, patient assignments, visit completion, staffing needs, and continuity of care.
• Lead daily clinical meetings to keep the team connected, address patient needs, identify barriers, and coordinate priorities. (In person once per week and virtual the other four days)
• Serve as a clinical resource for staff, assisting with complex patient situations, care planning, documentation, physician communication, and other clinical needs.
• Monitor patient care and clinical documentation to promote quality, regulatory compliance, timely billing, and positive patient outcomes.
• Manage and develop team members through regular feedback, joint visits, case reviews, competency assessments, performance evaluations, recognition, and ongoing education.
• Partner with Recruiting and leadership to identify staffing needs and participate in interviewing, selecting, onboarding, and retaining clinical team members.
• Monitor clinician schedules, productivity, and caseloads while balancing patient needs, staff capacity, and appropriate use of per diem resources.
• Support quality and safety initiatives, including incident review, infection control, patient feedback, staff education, and performance improvement efforts.
• Collaborate with Administrative, Marketing, and other clinical leaders to support referrals, census growth, efficient operations, and a positive patient and employee experience.
• Maintain clinical competency and provide patient or joint visits when needed to support patients and team members.

Qualifications:

• This position requires candidates to hold an active RN, PT, or OT license in the state of Massachusetts.
• Maintain current BLS CPR certification.
• Minimum of two (2) years of direct clinical experience; home health preferred.
• Prior supervisory experience preferred.
• Excellent written and verbal communication skills.
• Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.
• Ability to foster a positive and purposeful culture.
