Job summary

Amedisys is seeking a registered nurse clinical manager to lead clinical operations across home health and hospice services in the United States. You will supervise nursing staff, coordinate patient referrals and admissions, and oversee the Plan of Care to maintain high-quality patient outcomes.

Qualifications

• Associate or bachelor’s degree in nursing.
• Current RN license for the assigned state(s).
• One year of clinical RN experience in home health or hospice.
• One year of RN management experience.
• Current CPR certification.

Responsibilities

• Recommend employment decisions for staff to the director of operations, including hire/fire/promote or change status.
• Responsible for overall quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews requests for services and determines patient eligibility/suitability for home care services; coordinates referrals and admissions with intake.
• Oversees scheduling and payor verification/precertification requirements.
• Reviews documentation and ensures missing or untimely documentation is resolved.
• Assists clinicians in establishing immediate and long-term patient goals and Plan of Care.
• Evaluates patient needs and revises the plan of care with clinicians as needed.
• May provide direct patient care in emergencies.
• Participates in on-call process.
• Assists with training of staff to ensure quality care and fiscal responsibility.
• Participates in investigations of concerns from patients/families/physicians.

• Assists clinicians in establishing immediate and long-term patient goals, setting priorities and developing Plan of Care (POC).
• Regularly evaluates the needs of the patient and makes necessary revisions to the plan of care in collaboration with clinicians.
• May provide direct patient care on an infrequent basis in times of emergency.
• Participates in the on-call process.
• Assists with training of staff to ensure quality care, compliance and fiscal responsibility.
• Participates in the investigation and resolution of patient/family/physician concerns.

Qualifications

• Associate or bachelor's degree in nursing.
• Current RN license, specific to the state(s) you are assigned to work.
• One year of clinical RN experience in home health or hospice.
• One year of RN management experience.
• Current CPR certification.
