Job summary

Alternate Solutions Health Network is seeking a Clinical Manager to oversee clinical staff and patient care services across field and office settings. You will monitor compliance, ensure safe delivery of services, and follow state guidelines for licensed personnel.

Responsibilities include coordinating with intake, managing referrals, supervising on-call staffing and reviewing field productivity. A strong RN or PT background and leadership skills are essential.

Qualifications

  • Minimum of three years of RN or PT experience in acute care.
  • Current RN or PT license in the state of employment; RN preferred.
  • Strong critical thinking and communication skills; customer focus.

Responsibilities

  • Ensure quality and safe delivery of home health services.
  • Collaborate with intake, direct staffing and referrals based on availability.
  • Oversee Start of Care compliance and field liaison workflows.
  • Manage on-call rotation, staffing, PTO and productivity reviews.
  • Maintain licensing requirements and documentation per agency and Medicare guidelines.
  • Conduct field visits and training as defined by Administration.

The Clinical Manager is responsible for monitoring compliance, quality and safe delivery of services by following clinical processes. The Clinical Manager is responsible for any and all practices and duties within the scope of practice as outlined by the state. Provides backup to the Administrator/DON as needed.KEY RESPONSIBILITIESEnsure quality and safe delivery of home health servicesCollaborates with intake, providing daily direction, managing referral volume based on staffing availabilityProvides oversight for liaison workflow to ensure timeliness of completion as well as identify education needs for field & liaisons, ensuring workflow needs and staffing skill sets are congruent.Ensure Start of Care complianceFocus on Outcome Monitors as assignedImplement quality improvement programs in the fieldParticipates in all liaison/administrative on-call rotation, and patient staffing as needed to manage after hour’s team, staffing needs and patient related issuesManages on call calendar for internal & field staffManages PTO, call offs & daily/weekly productivity review to ensure proper staffing coverage to meet the needs of the business in the field and internallyCompletes field visits as neededParticipates in managing cost of good working with monitoring supply costs, overtime, staffing, bonuses and visits per episodeCollaborates with billing department to address authorization pro bono requests & ensures patient contact regarding authorization exceptionsPerforms new and ongoing liaison training as defined by the AdministratorManages staffing budget, at the direction of the Administrator to include: overtime & bonus oversight; reporting budget exceptions to Administrator; payroll entry of field staff bonuses; review and approve liaison timecardsProvides Administrator/DON with objective data related to internal and field employee performance evaluationsMaintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and familiesAssist with monthly Case Manager, Nurse and Therapy meetingsReviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesProcess complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator for resolution & follow upCompletes field clinician check-in reviews timelyMaintains active RN or PT license in state of employmentParticipates in in-service program developmentConsistently demonstrates core values and maintains a professional appearance as a representative of the companyUnderstands, implements and practices policies and procedures for agency operationMay act as back-up Administrator, as neededArrives at assigned location on scheduled work day. Works according to designated hours and on-call as needed in office and field nursing responsibilities.Current applicable license as an RN or PT. Knowledgeable with nursing/therapy best practices and the scope of practice for a nurse/therapist as outlined by the state for its licensed nursing/therapy personnel.Utilizes computer and phone continually to maintain constant contact with field staff, physicians, referral sources, internal staff and other departments.Knowledge of ASHN billing, authorization, intake, clinical review & workflow processes.Performs assigned duties with use of computer and phone.Completes and submits all required documentation within specified company requirements.Dexterity & vision to complete documentation on computer.Other duties as assigned.MANAGEMENT RESPONSIBILITIESResponsible for the day-to-day management of field staffInterviewing, hiring, training and termination of department personnelProvides consistent coaching and provides challenge and opportunity to direct reportsSupervises direct reports with consistency and clarity of expectationsCompletes yearly evaluations in an accurate and timely mannerPerforms coaching, counseling and discipline as neededWorks with direct reports to ensure employee satisfaction and resolution of issues and concernsTimecard managementQUALIFICATIONSMinimum of Three (3) years of experience as an RN or PT in an acute care settingStrong critical thinking skills with the ability to problem solve clinical needsAbility to create positive impressions and communicate with a variety of peopleExemplary communication skills and customer focusAbility to remain calm, have patience and be accommodatingCompassionate and caring while working with patientsAbility to resolve conflict using conflict management skillsKnowledgeable on nursing best practicesAbility to make appropriate nursing judgmentsAbility to identify a situation and handle it with the best possible solutionDetail-oriented and observant.Ability to resolve tough decision-making situations, providing direction to internal & field staffAbility to prioritize and be timelyAbility to clinically triage the needs of the department to ensure team goals are met timelyTakes advantage of available resources (individuals, processes, departments, and tools) to complete work effectively and efficientlyApplies fact-based knowledge of technical skills and business understandingAbility to follow directions and work as a team memberValid driver’s license and auto insurance.EDUCATION AND CREDENTIALSCurrent RN or PT license in the state of employment; RN is strongly preferredWe’ll help you put your passion for patient care to work.
