Bella Terra Home Health & Hospice is seeking a Clinical Manager/Field Nurse to join our team in Coachella Valley and Moreno Valley. This role blends clinical management with field nursing duties to ensure coordinated, high‑quality home health and hospice care.

Qualifications

  • Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred.
  • Two years management or supervisory experience in hospice/home care.
  • Experience working in an interdisciplinary setting.
  • Current RN license in the state of practice.
  • Adheres to professional standards and practice.
  • Excellent observation, nursing judgment and communication skills.

Responsibilities

  • Coordinate and manage patient care coordination and supervision of clinical personnel.
  • Assign hospice/home care personnel to cases as needed.
  • Review cases through home visits, conferences, and chart reviews to ensure quality care.
  • Assist in setting priorities and developing plans of care.

Position: Clinical Manager/Field Nurse
Pay Range: $120,000-$135,000 annually
Status: Full-Time
Coverage Area: Coachella Valley and Moreno Valley

JOB SUMMARY

The Clinical Manager/Field Nurse is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. This is a role that combines Clinical Manager duties with field nurse duties.

DUTIES & RESPONSIBILITIES
  • Receives case referrals.
  • Reviews available patient information related to case, including home visits, to determine hospice/home care needs.
  • Assigns appropriate hospice personnel to case as needed.
  • Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long‑term therapeutic goals, in setting priorities, and in developing plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel.
  • Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals.
  • Assists in the orientation of new employees.
  • Assists Clinical Director and other supervisory hospice personnel in the planning, implementation and evaluation of in‑service and continuing education programs.
  • Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.
  • Complies with accepted professional standards and principles.
  • Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs.
  • Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system.
  • Promotes customer service orientation to all hospice personnel.
  • Participates actively in quality assessment performance improvement program.
  • Performs other duties and activities as delegated by the Clinical Director.
  • Develop and initiate the patient’s individualized plan of care and lead ongoing, interdisciplinary assessments in collaboration with the patient, their representative, and caregivers.
  • Implement appropriate preventative, therapeutic, and rehabilitative nursing interventions as ordered by the physician and outlined in the plan of care.
  • Provide thorough patient and caregiver education, including disease management, medication management, safety, and home‑based care techniques.
  • Identify discharge planning needs early in the care process and ensure all necessary steps and education are completed prior to discharge.
  • Coordinate delivery of medical equipment, supplies, and additional services to ensure patients have what they need for safe, effective care at home.
  • Supervise Licensed Practical/Vocational Nurses (LPNs/LVNs) and Home Health Aides (HHAs), ensuring care is delivered in accordance with regulatory and organizational standards.

JOB REQUIREMENTS
  • Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred.
  • Previous experience in hospice/home care setting and with two years management or supervisory experience.
  • Proven ability to work within an interdisciplinary setting.
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Complies with accepted professional standards and practice.
  • Has excellent observation, good nursing judgement and communication skills.
  • Understands hospice philosophy and issues of death/dying.
  • Understands principles of pain/symptom management.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Complies and maintains current CPR certification.
  • Demonstrates excellent observation, verbal and written communication skills.
