Job description

Job Description: Clinical Services Manager

This is a remote/work-from-home position with travel required for member visits up to 75% of the time. The candidate must reside in Asherton, Big Wells, Pearsall, Dilly, Cotulla, Crystal City, or Carrizo Springs, covering surrounding areas including Uvalde and Del Rio. A valid LPN/LVN license or licensed social worker is strongly preferred.

Requirements:

  • Meet quality assessment standards of 92% or above
  • Complete documentation for assessments within 3 business days
  • Travel 75% of the time, with mileage reimbursement for member visits
  • Work schedule: 8am-5pm, Monday-Friday, with possible overtime

Duration: 3+ months, with potential for contract-to-hire

Responsibilities:
  • Travel 3 days a week to assess members in their homes and 2 days remote for documentation and follow-up
  • Assess members for approval or denial of personal attendant services using H2060 form
  • Evaluate complex, high-risk members and develop care plans
  • Act as liaison and advocate for members and their families
  • Educate members and caregivers on healthcare topics and coordinate community resources
  • Ensure appropriate referrals and access to services
  • Maintain accurate documentation in the electronic system, ensuring compliance with guidelines
  • Provide feedback for quality improvement
  • Perform additional duties as assigned and adhere to policies

Qualifications:

  • Experience working with the aged, blind, or disabled population for 5+ years
  • Bilingual skills strongly preferred
  • Background in Social Services, Social Work, Care Management, Nursing, LVN, or Medical fields
  • Educational requirements: Bachelor's degree with 2-4 years of related experience or LVN with similar experience
