Job Description: Position Purpose:
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.

Education/Experience:
Requires a Bachelor's degree and 1 year of related experience or equivalent experience reflecting the duties and skills of this position.

Specific Requirements for Certain Populations:

  • Iowa Plan Only: Bachelor's with 30 semester hours in a human services field and one year of relevant experience, or an Illinois-licensed RN with similar experience.
  • Illinois Plan Only: For Physically Disabled/Elderly, candidate must meet one of three criteria: RN licensed in Illinois, a degree in a human services field, or LPN with one year of relevant experience. For Brain Injury/HIV/AIDS, criteria include RN with a degree or experience, licensed or unlicensed social workers with specified degrees, and experience working with specific populations.

Key Responsibilities:

  • Evaluate member needs, resources, and develop or facilitate care plans.
  • Identify providers, specialists, and community resources for long-term care.
  • Coordinate between members, families, caregivers, and providers to ensure service accessibility.
  • Provide resource support for various needs based on assessments.
  • Monitor and update care plans, and interact with healthcare providers to meet member needs.
  • Maintain documentation for compliance with regulations.
  • Perform on-site assessments as needed.
  • Educate members and families on procedures, services, and benefits.
  • Provide feedback for quality improvement.
  • Perform other duties as assigned and ensure policy compliance.
