Job summary

Beth Israel Lahey Health is seeking an LPN to oversee medication management and coordinate medical needs for residents at The Herrick House. You will observe changes in condition, document findings, and coordinate with physicians and families.

You will supervise Resident Care Aides and ensure care is delivered per plan. Qualifications include graduation from an approved School of Nursing, MA RN license, and BLS certification, with at least 1 year of relevant nursing experience.

Qualifications

• Education: Graduate of a state approved and/or accredited School of Nursing.
• Experience: A minimum of 1 year of medical/surgical nursing or nursing in a skilled nursing facility is required.
• License/Certification(s): Current license to practice nursing from the Massachusetts Board of Registration. BLS certification.

Responsibilities

• Oversees the medication management and coordinates the medical needs of the residents with physicians, Director of Nursing/Resident Care and family members.
• Observes, documents and reports significant changes in residents' condition recommending changes in care needs.
• Assists in supervising Resident Care Aides ensuring care is provided as instructed.
• Collaborates with nurses, physicians, and families to ensure quality and continuity of care.
• Leads and participates in teaching, counseling, and care planning to meet patient needs.
