Responsibilities:

  • Meet with patients in hospitals or nursing facilities to discuss services and facilitate transition to home health or hospice care.
  • Act as a liaison for clinical appropriateness to internal Branch Directors regarding patient discharge complexity and service needs.
  • Coordinate with physicians to arrange home health services, including taking written and verbal orders.
  • Partner with field-based Account Executives to provide clinical education to referral sources.
  • Review hospital hold lists and participate in sales meetings and case conferences.

Qualifications:

  • Associates or Bachelor’s degree in nursing with LVN/LPN license.
  • 1-3 years of relevant experience.
