Description

This is a part-time (28 hours/week) position that will be floating between PPMC and PSVMC.

This position represents Providence Home Services (HS) at contracted facilities and requires strong communication and interpersonal skills. The Clinical Liaison coordinates and arranges home care services between HS and contracted facilities for home health, home infusion, and home medical equipment related to clinical referrals, except when an onsite HME liaison is present.

The role acts as a clinical resource, educating hospital staff about HS services, and contributes to the patient care plan, especially regarding discharge planning. It ensures a smooth transition for patients and their families post-hospitalization.

Required Qualifications

• Oregon Registered Nurse License (upon hire)
• National Provider BLS - American Heart Association (upon hire)
• Criminal background check in accordance with Providence HR policy #302

Preferred Qualifications

• Bachelor's Degree in Nursing
• One year of full-time experience in home health or hospice
• One year of full-time experience in home infusion
• One year of full-time experience in discharge planning or care management
• Experience in home care and customer service skills
