Summary of Duties
Responsible for skilled nursing clinical admission using independent judgment and discretion. Coordinates admission process between physicians, referral sources, and facility staff to efficiently evaluate and admit residents. Develops and implements marketing strategies for referral source development.
Essential Job Functions
- Screen and assess appropriate referrals for admission and discuss potential for admission with staff.
- Direct and process admissions, serving as the liaison between the Directors of Admissions, hospitals, physicians, and referral sources.
- Develop a system for coordinating and monitoring resident assessments and referrals.
- Conduct and document nursing assessments as necessary.
- Complete and submit admission assessment reports as requested.
- Coordinate admissions with staff to ensure smooth flow of information and activity.
- Serve as a point-of-contact for referrals/inquiries and conduct initial clinical reviews as needed.
- Process referrals and inquiries.
- Work with other departments to ensure an accurate and timely admission process.
- Maintain an accurate record of available beds/units.
- Communicate resident care and medical equipment needs upon admission with the Director of Admissions.
- Conduct rounds and inform staff to ensure readiness for admissions.
- Obtain necessary medical records for admission, such as diagnosis, physician orders, OBRA screening, etc.
- Assume additional responsibilities and special projects as assigned.

- Ensure residents and families understand the level of care, expectations, and billing procedures.
- Communicate resident progress to staff, physicians, and referral sources as needed.

- Orient residents and families to payment sources, rates, and resident care procedures.
- Treat residents with kindness, dignity, and respect.
- Work with residents, physicians, and staff professionally and courteously, supporting residents and their families.
- Maintain a clean and safe work environment.
- Maintain confidentiality and respect residents' privacy and dignity.
- Collaborate with staff to achieve departmental and organizational goals and promote good relations.

- Participate in facility and divisional meetings.
- Report resident abuse, neglect, or misappropriation promptly.
Education
Must have completed an accredited RN or LPN program and be licensed in Alabama.
Experience
Minimum of two years in case management and discharge planning.
