Job summary

Evergreen Healthcare Group is seeking a Clinical Liaison to serve skilled nursing facilities in the Portland/Hillsboro area. In this regional role, you’ll connect hospitals, families, and facilities to coordinate admissions and post-acute transitions with clarity and compassion.

Active LPN or RN license preferred; nursing background and strong communication are essential. Expect travel between facilities with mileage reimbursement, private healthcare, and professional development opportunities

Qualifications

Active LPN or RN license preferred.
Experience in skilled nursing, post-acute, or discharge planning.
Excellent communication and relationship-building skills.
Willingness to travel between facilities and reside within commuting distance of Portland/Hillsboro, OR.

Responsibilities

Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers.
Evaluate patient referrals and assist with clinical screenings and pre-admission assessments.
Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care.
Serve as a clinical resource and liaison to families and referral sources.
Collaborate with internal facility teams to align care plans and meet resident needs.
