Job summary

Spectrum Healthcare Solutions is seeking a Nurse Liaison (LPN / Medical Assistant) to coordinate services across long-term care, skilled nursing, inpatient rehab, and assisted living facilities. The role bridges the gap between medical providers and facility staff to improve patient outcomes and reduce hospital readmissions.

The position is highly autonomous with a strong clinical and administrative component.

Qualifications

• Must have Medical Assistant or LPN knowledge and clinical background.
• Strong working knowledge of medical terminology and disease processes.
• Exceptional organizational skills and attention to detail for data collection and reporting.

Responsibilities

• Manage daily physician rounding within assigned facilities.
• Prepare equipment and information two hours before rounding.
• Assist physician during examinations and enter orders into the EMR.
• Reconcile medications and report lab and imaging findings to the physician.
• Transcribe and prepare physician notes in the EMR within 24 hours.
• Serve as primary liaison between staff, providers, and patients.
• Coordinate with Nurse Practitioners and facility staff on orders.
