This temporary position will be traveling and covering the Santa Monica area.
The Clinical Liaison position is responsible for creating and maintaining positive customer relationships to increase referrals for services such as Home Health, Hospice, Home Infusion, PACE, and Transitional Care Units. This role involves working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, coordinate services, and ensure proper documentation for compliance with agency and statutory guidelines to manage patient transitions from acute care to home or other residences.

This role offers a pay range of $35.06 - $54.43 per hour, with potential additional compensation such as shift differentials, on-call pay, overtime, and bonuses.
