Description

This temporary position will be traveling and covering the Santa Monica area.

The Clinical Liaison position is responsible for creating and maintaining positive customer relationships to increase referrals for services such as Home Health, Hospice, Home Infusion, PACE, and Transitional Care Units. This role involves working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, coordinate services, and ensure proper documentation for compliance with agency and statutory guidelines to manage patient transitions from acute care to home or other residences.

Required Qualifications

• Education sufficient for clinical licensure for discipline (LPN/LVN) OR
• Master's Degree in Social Work
• Current state professional license or certification appropriate to discipline upon hire
• At least 3 years of experience in a clinical discipline

Preferred Qualifications

• Liaison, marketing, or sales experience
• Healthcare industry experience
• Documented successful sales account medical management experience
• Epic EMR experience
