  • Identify trends or barriers to admission targets and communicate to management.
  • Develop and maintain collaborative relationships with internal and external staff for customer satisfaction.
  • Nurture referral source relationships to ensure satisfaction.
  • Support challenging transfers with SR/MGB HC resources.
  • Consider discharge plan alternatives to meet patient needs.
  • Coordinate staffing assignments and reallocate resources to meet needs.

The liaison coordinates the referral process to ensure a smooth transition of patients to Mass General Brigham Home Care. The Liaison ensures the patient meets criteria for admission, is clinically appropriate, and is ready to transfer to a post-acute provider. Perform preadmission patient evaluations when required.Does this position require Patient Care?YesEssential Functions-Orient and mentor new team members thoroughly in liaison processes.-Monitor staff activity and provide feedback.
