This position provides certified home health and hospice care coordination and discharge planning for patients referred by the hospital’s case managers to support the delivery of effective and efficient home health and hospice care. Collaborates with members of the health care team to identify appropriate utilization of resources and to ensure maximum reimbursement. Serves as a liaison with patients and the health care team to ensure a smooth transition from hospital to home health and/or hospice care. Maintains knowledge of current laws and regulations of federal, state, and regulatory bodies relative to home health and hospice and works within those constraints to maintain actions within the agency’s and company’s compliance programs.

I. Major Responsibilities
- Provides initial introduction to the patient/family, explains the Care Transitions Programs, and provides patient with contact information.
- Completes initial screening of all referred patients utilizing specific criteria to identify certified home health and/or hospice needs at time of discharge.
- Utilizes a combination of clinical information, screening criteria, and third-party information to evaluate patients for appropriateness of admission type and setting.
- Collaborates with third party payers to ensure appropriate reimbursement for home care/hospice.
- Identifies potential barriers to discharge or transfer based on established criteria. Communicates potential barriers to the care team to spearhead resolution of any issues. Reports issues and system barriers to efficient and effective discharge planning to leadership.
- Consults with medical staff as needed to facilitate accurate documentation and ensures appropriate documentation is obtained from the patient’s physician prior to discharge. Documents in the patient’s record according to department policies and procedures.
- Identifies cases that fail to meet established criteria and promptly reviews with the assigned case manager and physician to resolve the issue. Elevates complex cases to leadership and/or the physician as appropriate.
- Facilitates a patient-specific, safe discharge plan to a home health or hospice program.
- Contacts patients after discharge to ensure compliance with and understanding of the discharge plan.
- Visits and establishes effective business relationships with physicians, discharge planners, and other referral services.
- Delivers and participates in health educational services provided to staff, physicians, and patients.
License/Certification/Education
II. Position Qualifications:
Required
- Completion of an accredited Licensed Practical Nurse Program.
- Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
Preferred
Experience/Skills
Required:
- Minimum five (5) years as a Clinical Liaison or similar role, predominantly in hospice and certified home health.
- Ability to build and maintain effective relationships with potential referral resources.
- Strong verbal and written communication skills.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
