Job summary

Trilogy Health Services, LLC is seeking a Clinical Liaison to support a cluster of health campuses. You will develop and manage communications between patients, families, and care professionals to drive admissions from hospitals to Trilogy campuses.

The role includes pre admission assessments and determining eligibility for care. You will collaborate with discharge planners, educate hospital staff, and build community relationships to foster partnerships and ensure patients are matched to the

Qualifications

• Associate’s degree required; hospital/healthcare experience preferred.
• Experience in discharge planning, case management, or utilization management.
• Experience in a hospital, medical facility, or skilled nursing setting preferred.

Responsibilities

• Collaborates with discharge planners to prioritize referrals and identify patients for transition to a Health Center campus.
• Establishes patients’ eligibility for care, communicates with families, and interacts with staff from case managers to physicians.
• Conducts pre‑admission assessments to gather clinical documentation and reviews it for insurance approval requirements.
• Responds promptly to patient referrals from hospital case managers, physicians, and managed care organizations.
• Educates hospital staff about Trilogy Health Services’ capabilities and matches patients to facilities and services based on needs.
• Educate patients and families about services offered and explain available care options prior to admission.
• Develops relationships within the community on behalf of Trilogy Health Services and its campuses.
• Participates in hospital and ACO meetings to foster partnerships with key contacts.
