Qualifications

• 1–3 years of relevant experience preferred related to discharge planning or case management.
• Previous experience in a hospital, medical facility, or skilled nursing care setting is required.
• Valid and unencumbered driver's license is necessary.

Responsibilities

• Collaborates with discharge planners to prioritize referrals.
• Establishes patients' eligibility for care and communicates with families.
• Conducts pre-admission assessments and gathers clinical documentation.
• Responds immediately to patient referrals from hospital case managers.
• Educates hospital staff about Trilogy's clinical capabilities.
• Develops relationships within the community on behalf of Trilogy Health Services.
