Job summary

Elara Caring is seeking a Sales Clinical Liaison to support care transitions and foster relationships with hospital staff and key accounts. You will identify referrals, collaborate with the AVP of Sales, and coordinate with care teams to position patients for the right care in the home setting.

Qualifications

  • 3+ years of practical experience in a clinical setting.
  • LPN/RN license preferred.
  • Bachelor’s degree preferred.
  • Reliable vehicle, valid license and auto insurance per state laws.
  • High school diploma or GED required.

Responsibilities

  • Identifies and collects referral information to support eligibility for care.
  • Makes live calls with the AVP of Sales to foster relationships with key account staff.
  • Calls on Key Accounts per the AVP and QBR plan.
  • Explains care options to patients and families.
  • Coordinates with hospital staff to position the patient for the right care at the right place.
  • Develops positive relationships in the community.

As Sales Clinical Liaison, you’ll contribute to our success in the following ways:
  • Identifies and collects referral information supporting the patient’s eligibility for care.
  • Makes “live” calls in partnership with aligned Area Vice President of Sales to foster relationships with key account staff.
  • Calls on Key Accounts assigned by AVP in partnership with Account Executive and Quarterly Business Review (QBR) plan.
  • Makes calls/ referral collaboration to service these referral opportunities.
  • Communicates with the families of patients regarding services offered.
  • Interacts with a variety of hospital staff from different departments.
  • Consolidates and reviews patients’ records upon referral, as part of the discharge process.
  • Collaborates with hospital staff to help position the patient for the right care at the right place.
  • Explains care options to patients and their families.
  • Works with a healthcare facility as needed to collaborate care as part of discharge planning for referred cases.
  • Coordinates with healthcare facility staff to ensure that each patient has a smooth transition through the discharge and back to the home environment.
  • Collaborates with Intake for insurance eligibility and coverage.
  • Coordinates communication between patients’ and their physicians, case managers and other medical staff.
  • Develops positive relationships in the community.
  • Performs other duties/projects as assigned.
What is Required?
  • High School Diploma/GED
  • 3+ years’ practical experience
  • LPN/RN License is preferred
  • Bachelor’s Degree is preferred
  • Experience in Case Management in a clinical setting is preferred.
  • Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State
  • You will report to the Area Vice President Sales.
