Mass General Brigham’s Spaulding Rehabilitation Hospital is seeking a Per Diem Community Outreach Liaison to advance patient care through local partnerships. You will build and maintain relationships with community organizations, physician practices, clinics, senior living facilities, and other providers to promote hospital services and create referral pathways.

Qualifications

• Bachelor's Degree in Healthcare Management or related field.
• Experience in community outreach preferred.
• Active, clinical license in MA required (RN, PT, OT, SLP, LICSW, LMHC, etc.).

This is a per diem position working on an as needed basis for coverage purposes based.

Summary

Responsible for establishing and maintaining strong connections with community partners, healthcare providers, and relevant organizations by promoting the hospital's services, building relationships, and facilitating seamless patient transitions between the community and hospital settings.
