Genesis Healthcare is seeking a Director of Post-Acute Partnerships to expand referral networks, strengthen hospital collaborations, and drive census growth across skilled nursing centers. Qualifications

• Five+ years of clinical, healthcare sales, business development, case management, or post-acute care experience.
• Experience with hospitals, managed care organizations, or skilled nursing facilities is highly desirable.
• RN or LPN license preferred (where applicable), but not required.
• Excellent communication, presentation, and relationship-building skills.
• Ability to analyze data, develop strategic plans, and achieve measurable performance goals.

Responsibilities

• Develop and strengthen relationships with hospitals, physicians, case managers, discharge planners, accountable care organizations, and community partners.
• Identify new referral opportunities while maintaining and growing existing partnerships.
• Coordinate patient referrals from initial clinical review through admission, ensuring seamless transitions of care.
• Collaborate with admissions, nursing, and executive leadership to deliver outstanding patient and customer experiences.
• Promote the clinical capabilities and specialty programs that set our centers apart.
• Leverage CRM technology and market data to identify growth opportunities and measure performance.
• Create and execute strategic business development plans that increase admissions and market share.
• Analyze referral trends, conversion metrics, and market intelligence to continuously improve results.

Qualifications

Preferred qualifications include:
