Job summary

Druid City Infusion is seeking a Clinical Liaison to coordinate patient transitions from facilities to home settings and to support referral sources as part of the Vital Care Infusion Services network. You will work with Account Executives to promote new referrals and ensure smooth care transitions for patients in communities we serve.

Requirements include at least two years of nursing experience with case management or home health, plus strong interpersonal and customer service skills.

Responsibilities

• Manage daily and weekly schedule of patient care transition coordination, patient assessment and teaching, and marketing support activities.
• Provide patient education teaching sessions in the facility.
• Maintain effective working relationships with facility referral sources.
• Seek out new facility referral source contacts and opportunities for additional referral activity from existing referral sources.
• Collaborate with the Account Executives in the market for business development activities.
• Complete clinical and patient care coordination documents timely and accurately.
• Assist Nurse Manager or General Manager with education for Home Health Agency staff.
• Demonstrates positive, informative interaction with other departments, co-workers, referral sources and hospital physicians.
• Adheres to all applicable policies, procedures, safety plans and standards imposed by regulatory organizations and Vital Care.
• Coordinates referral source and patient activities closely with the Account Executive(s) responsible for the customer account.

The Clinical Liaison (CL) performs daily marketing and patient care transition coordination activities for various facility referral sources, case managers and patients referred to Vital Care. The overall focus of the CL is to work in concert with the Account Executive(s) and Operator to maintain the existing customer base, promote new referral opportunities and coordinate a smooth transition of care of the patient from the facility or physician’s office to the home setting.

Qualification

• A minimum of two (2) years of previous nursing experience, with case management or home health preferred
• Previous marketing experience preferred
• Excellent interpersonal and customer service skills

Education

• Licensed Registered Nurse in the state assigned
• Bachelor's degree in nursing from an accredited school
• Current license to practice nursing in the state or territory assigned

Physical Requirements

Daily driving throughout assigned territory. Ability to travel up to 10% of time. While performing the duties of this job, the employee is regularly required to sit and talk or listen. The employee is frequently required to walk and use hands to finger, handle or feel. The employee is occasionally required to stand and reach with hands and arms. The employees must frequently lift and/or move up to 10 pounds and occasionally lift and or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus. The noise level in the work environment is usually moderate.
