Position Overview
The Clinical Intake/Referral Specialist is responsible for managing the intake process for patients needing home health care services. This role involves gathering information from healthcare providers, ensuring smooth administrative processes, and confirming that all necessary documentation is complete before patients begin treatment or care.
Schedule: Monday - Friday, 9 am - 5 pm

Essential Duties & Responsibilities
- Assist referral sources in evaluating the service needs of clients and families.
- Process detailed information from referral sources prior to initial home assessments to facilitate a smooth transition to home care.
- Evaluate crisis calls for immediate intervention as needed.
- Interpret agency policies and capacity to meet service requests.
- Maintain current knowledge of community resources.
- Communicate intake information to clinical teams.
- Keep management informed of referral volume and flow.
- Possess working knowledge of Medicare/Medicaid and private insurance related to home care benefits.
- Participate in orientation programs for new staff.
- Perform any other duties assigned by supervisors.
- Document patient and family concerns or complaints on incident reports.
On Call Responsibilities
- Answer all calls from the answering service.
- Take referrals, MD orders, and lab results during non-business hours and relay information to the appropriate department in the morning.
- Triage patient calls to visiting on-call nurses or hospitals, or notify the day supervisor of issues requiring next-day visits.
- Document all calls and their status.
- Record nursing information or patient education in clinical narratives.
- Perform any other tasks as assigned by the supervisor.
Qualifications
- Current Driver’s License
- Education:
- Graduate of NLN CNEA-accredited LPN program
- Current Massachusetts LPN license
- Experience:
- Two years of progressive experience in home health care preferred.
- Skills:
- Ability to read and interpret safety rules, operating and maintenance instructions, and procedure manuals.
- Proficiency in MS Word, Outlook, and Excel.
