New Horizons is seeking for a Clinical Intake Counselorwho will be responsible for conducting comprehensive biopsychosocial assessments, developing individualized treatment plans, completing risk assessments, crisis de-escalation and administering mental health and substance use outcome measures. This role ensures clients are connected to the appropriate level of care and linked to relevant community resources. The therapist will work with clients across the lifespan and support both scheduled and walk-in appointments. In addition, this position provides clinical follow-up services for clients discharged from Crisis Stabilization Units and those referred for continued care following medication management appointments. May include rotating coverage for crisis follow-up or coordination with Crisis Stabilization Units.
