Job summary

Aurora Health Care in Wisconsin is seeking a licensed pharmacist to oversee pharmaceutical care, review medication orders, and design therapy regimens in collaboration with the health care team.

You will educate patients, ensure safety and compliance, participate in formulary management, precept students, and support quality improvement initiatives in a fast-paced clinical setting.

This role requires ACPE-accredited degree or NABP FPGEC and state licensure; experience is not required.

Responsibilities

  • Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
  • Uses effective patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, compliance, and administration.
  • Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens based on biomedical literature and organizational policies.
  • Reviews and interprets medication orders and prescriptions for accuracy.
  • Participates in management of medical emergencies when applicable.
  • Supports medication safety and quality improvement initiatives and reports events through proper channels.
  • Performs QA and performance improvement of the medication-use system.
  • Collaborates with the healthcare team on formulary adherence and non-formulary medication acquisition.
  • Complies with policies, procedures, and regulations; ensures proper storage and handling of medications.
  • Serves as a preceptor for pharmacy students and residents; mentors staff and students as assigned.
  • Directs and checks the accuracy of work by pharmacy support staff to ensure safe, efficient workflows.
