Job summary

VNS Health in New York is seeking a Clinical Field Manager to lead a diverse group of home care clinicians, ensuring patient care aligns with our policies and industry standards. You will guide a team from admission to discharge and support seamless integration into VNS Health Home Care.

You will oversee staffing, recruitment, scheduling, and the use of clinical tools to elevate outcomes, while fostering growth within a dedicated care team.

Qualifications

• RN license and current registration in New York State
• Bachelor’s degree in nursing (required) or equivalent
• Master’s degree in nursing or equivalent (required)
• Minimum of three years nursing experience and one year home health experience
• Clinical management experience, required

Responsibilities

• Oversee an interdisciplinary team of community-based clinicians that deliver quality patient care from admission to discharge.
• Manage, empower, and motivate team members in a dynamic work environment.
• Proactively execute administrative functions to achieve seamless patient care including the review of clinician caseloads, staffing needs, recruitment process, and assigned scheduling of team members.
• Use and learn tools and systems, problem solve new and different situations, and elevate issues timely to leadership to achieve organizational success and improved clinical outcomes.

You will be responsible for leading, coordinating, and overseeing a diverse team of dedicated interdisciplinary professionals, from early career to seasoned clinicians. As an integral part of the clinical management team, you uphold the mission, vision, and values of VNS Health, supporting a team that elevates patient-centered care to New Yorkers most in need. Qualifications

Licenses and Certifications:

• License and current registration to practice as a Registered Professional Nurse in New York State, required

Education:

• Bachelor’s degree in nursing from a program approved by the New York State Department of Education or equivalent work experience or
• Master's degree in nursing or equivalent work experience, required

Work Experience:

• Minimum of three years nursing experience and one year home health experience or
• Clinical management experience, required

Pay Range

USD $98,200.00 - USD $130,800.00 /Yr.
