Acentra Health is seeking Clinical Evaluators to join our growing team in Los Angeles, California. We currently have two open positions available in the Los Angeles area.
Job Summary
Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Los Angeles, California. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Los Angeles.
As a Clinical Evaluator, you will perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals in need. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes. This position offers the flexibility of working from your home office, combined with local travel. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field.
Job Responsibilities
Conduct PASRR screenings, evaluations, and determinations through comprehensive clinical assessments of patients, including review of relevant records and input from collateral sources.
Required Qualifications, Knowledge, and Experience
Master’s degree from an accredited college or university in a relevant field (e.g., psychology, social work, counseling, or a related discipline) and at least 2 years of experience conducting clinical assessments for individuals with mental health or psychiatric disorders, including complex differential diagnoses involving substance use, medical conditions, cognitive impairments, and dementia.
Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.
Solid understanding of standards of practice related to Level of Care determinations and familiarity with standards pertaining to mental health, geriatrics, and other conditions that often require long-term care placement.
Prior experience working with the geriatric population is preferred.

Compensation
The compensation for this role is $85,280.00 to $88,400.00base salary
Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
