Job summary

Kaweah Health is seeking a Registered Dietitian to provide medical nutrition therapy, collaborate with a multidisciplinary team, and deliver nutrition education to patients and families in Visalia, CA. The role covers diverse care settings including renal, diabetic, pediatric, and long-term care contexts.

The dietitian will develop care plans, monitor progress, and contribute to quality improvement initiatives within the Food & Nutrition Services department.

Qualifications

• Performs assessment and screening for high-risk patients.
• Develops nutrition care plans in collaboration with the interdisciplinary team.
• Provides nutrition information and recommendations to physicians.
• Delivers nutrition education to patients and caregivers.
• Charts in the patient’s medical record in a timely manner.

Responsibilities

• Performs assessment and screening for high-risk patients and develops nutrition care plans with the interdisciplinary team.
• Follows up to evaluate progress toward goals.
• Provides nutrition information to physicians regarding patient status.
• Charts appropriately in the patient’s medical record.
• Provides nutrition education to patients and/or caregivers and tailors to cultural/ readiness needs.
• Participates in Continuum of Care meetings and interdisciplinary rounds as nutrition consultant.
