Job summary

FutureCare is seeking a Registered Dietitian to join our clinical team in Maryland. You will collaborate with an interdisciplinary team to provide individualized nutrition care, perform assessments, and develop care plans for residents in a skilled nursing facility.

The role requires a Maryland RD license, a Bachelor's in Nutrition or Dietetics, and at least 2 years of dietitian experience, preferably in skilled nursing.

Qualifications

• Current Registered Dietitian license in the state of Maryland.
• Bachelor's degree in Food and Nutrition, Dietetics, or a related field.
• At least 2 years of experience as a Dietitian, preferably in a skilled nursing facility.
• Knowledge of the Nutrition Care Process and ability to apply all steps.
• Strong desire to deliver care in an empathetic, compassionate way.

Responsibilities

• Work with the Interdisciplinary Team to ensure residents receive the best clinical care.
• Provide nutrition services and medical nutrition therapy aligned with patient needs.
• Assess nutritional status using validated screening tools.
• Complete timely nutrition and MDS assessments as required.
• Plan, develop, implement and monitor the nutrition care plan for each resident.
• Provide nutrition recommendations for patients reliant on enteral nutrition.
• Educate patients and families on nutrition matters.
• Utilize current standards and evidence-based practices.
• Prioritize patient-centered care and residents’ rights.
