Heritage Grove Estates in Topeka, Kansas, is seeking a dedicated Registered Dietitian to lead residents' nutritional care, conduct assessments, and develop individualized nutrition plans. You will guide dietary practices and ensure high standards of dietary services across the facility.

The role requires active RD credentials, Kansas licensure, and experience in clinical nutrition and EMR systems. Join a supportive team focused on resident-centered care and outcomes.

Qualifications

• Current, active RD or RDN credential in good standing (CDR).
• Licensed (or eligible) as a Dietitian in Kansas.
• Bachelor’s or Master’s in Nutrition or related field from an accredited program.
• Completed accredited Dietetic Internship or supervised practice.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of clinical nutrition, therapeutic diets, MNT, and CMS regulations.
• Experience with EMR and nutrition documentation systems.
• Excellent communication and interpersonal skills; strong organization.

Responsibilities

• Provide nutrition assessments and develop individualized care plans.
• Deliver medical nutrition therapy per physician orders and guidelines.
• Monitor nutrition status, adjust plans based on labs and weight changes.
• Collaborate with nursing, dietary, therapy, and care teams for quality care.
• Oversee texture modifications and therapeutic diet implementation.
• Ensure compliance with federal, state, CMS regulations; documentation accuracy.
• MDS/RAI involvement and nutrition-related assessments in care planning.
• Support nutrition quality measures including hydration and malnutrition prevention.
• Educate residents, families and staff on nutrition needs and health management.
• Assist menu planning, food service operations to meet clinical standards.
• Maintain timely EMR documentation and stay updated on best practices.
