Job summary

Spring Creek Healthcare Center in Perth Amboy, NJ is seeking a Registered Dietitian to lead nutrition care for our diverse patient population. You will conduct diet assessments, develop personalized meal plans, and collaborate with the care team to support hydration and healthy eating.

You will document treatment plans, participate in Quality Improvement initiatives, and educate patients and families. Join a 179-bed facility and help improve residents' health and quality of life.

Qualifications

• Registered Dietitian RD.
• Bachelor's degree in Dietetics or related field.
• Registration with the Commission on Dietetic Registration (CDR).

Responsibilities

• Conducting diet assessments and developing personalized meal plans for patients with a variety of dietary needs and restrictions.
• Collaborating with the healthcare team to ensure that patients receive optimal nutrition and hydration.
• Providing education and support to patients and their families on nutrition and healthy eating habits.
• Maintaining accurate and detailed records of patient diets and treatment plans.
• Participating in Quality Improvement initiatives and contributing to a positive and supportive care environment.
