Overview

This job introduces the full scope of services provided by the Outpatient and Home Infusion Pharmacy to Clinical Staff. Ensures the ongoing updates and education for referral sources. Responsible for the successful clinical coordination of care from the referral source into the home setting. Provides and utilizes clinical expertise and skills to assist referral sources and/or patients with: identification of appropriate candidates for home infusion, identification of the patient\'s or caregiver\'s ability to provide care, patient education and training regarding therapy and supplies. Assumes responsibility for clarifying expectations of care in the home, medication connection at the hospital/facility in order to expedite the discharge into the home setting as needed. Completes nutritional assessments, prepares nutrition care plans, recommends nutritional therapies, and monitors the effectiveness of nutritional care as required and if applicable. Provides nutrition education to promote improved health and complies with regulatory requirements. Conducts and participates in community education, marketing, and public relations efforts. Participates in in-services and continuing education programs.

Education

Required -

Bachelor of Science degree in Nutrition, Dietetics or related field

Work Experience

Required -

1 year clinical work experience with knowledge of normal and therapeutic nutrition, nutrition screening and support principles, and modified diets.

Preferred -

(1) year of Home Infusion experience

Certifications

Required -

Registered Dietitian status with the Commission on Dietetic Registration (CDR) with the Academy of Nutrition and Dietetics. Current licensed Dietitian/Nutritionist status with LBEDN

Knowledge Skills and Abilities (KSAs)

  • Must have computer skills and dexterity required for data entry and retrieval of information.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally.
  • Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
  • Strong interpersonal skills.
  • Strong personal commitment to professional development and maintaining clinical services that are evidence based.
  • Excellent analytical skills to assess nutritional status and develop appropriate care plans based on problems identified.
  • Excellent organizational and prioritization skills to meet frequent deadlines.

Job Duties

  • Collaborates with referral sources (i.e. hospital case managers, home health agencies, physician offices, etc.) to facilitate the coordination of care for new patient referrals.
  • Coordinates and communicates all discharge planning activities for new patient referrals to appropriate personnel and referral sources
  • Assesses patients’ nutritional needs and creates nutrition care plans.
  • Provides nutritional education and counseling.
  • Provides education for other home infusion therapies as appropriate
  • Documents records completely and accurately.
  • Supports departmental goals.
  • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Other related duties as required.
