This job introduces the full scope of services provided by the Outpatient and Home Infusion Pharmacy to Clinical Staff. Ensures the ongoing updates and education for referral sources. Responsible for the successful clinical coordination of care from the referral source into the home setting. Provides and utilizes clinical expertise and skills to assist referral sources and/or patients with: identification of appropriate candidates for home infusion, identification of the patient\'s or caregiver\'s ability to provide care, patient education and training regarding therapy and supplies. Assumes responsibility for clarifying expectations of care in the home, medication connection at the hospital/facility in order to expedite the discharge into the home setting as needed. Completes nutritional assessments, prepares nutrition care plans, recommends nutritional therapies, and monitors the effectiveness of nutritional care as required and if applicable. Provides nutrition education to promote improved health and complies with regulatory requirements. Conducts and participates in community education, marketing, and public relations efforts. Participates in in-services and continuing education programs.

Education
Required -
Bachelor of Science degree in Nutrition, Dietetics or related field
Work Experience
Required -
1 year clinical work experience with knowledge of normal and therapeutic nutrition, nutrition screening and support principles, and modified diets.
Preferred -
(1) year of Home Infusion experience
Certifications
Required -
Registered Dietitian status with the Commission on Dietetic Registration (CDR) with the Academy of Nutrition and Dietetics. Current licensed Dietitian/Nutritionist status with LBEDN
