Charles River Community Health (CRCH) is seeking a Registered Dietitian Nutritionist to provide 2-4 clinical sessions per month, with potential to expand as demand grows. The role involves assessing nutritional needs, creating individualized plans, and delivering counseling to patients across diverse backgrounds at CRCH sites in Brighton and Waltham.

The ideal candidate holds Massachusetts RD/RDN credentials, 1-3 years of clinical nutrition experience, and strong communication skills.

Formación

• Bachelor's degree required if graduation prior to 2024.
• Massachusetts Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification.
• 1-3 years of clinical nutrition experience, preferably in a community health setting.
• Bilingual (Spanish/English, Portuguese/English) is strongly preferred.
• Certification in BLS required.
• Strong knowledge of evidence-based nutrition guidelines and preventative health practices.
• Must have knowledge base of nutritional needs of patients with obesity, diabetes, hypertension, heart disease and other medical conditions benefiting from dietetic and nutritional care.
• Experience supporting breastfeeding and lactating individuals, including providing lactation education, guidance, and referrals preferred.
• Knowledge of evidence-based lactation and infant feeding practices preferred.
• Experience facilitating group education or workshops preferred.
• Willingness to educate and mentor clinical support staff as needed.
• Must have basic computer skills.
• Must have a willingness to work flexible hours to meet the organization's needs/demands.
• Must be able to travel to either Charles River Community Health site (Brighton and Waltham) as needed.
• Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
• Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.

Responsabilidades

• Provide culturally sensitive, patient-centered care of the highest quality to patients of all backgrounds and income levels.
• Assess nutritional and health needs of patients of all ages by reviewing past medical history, obtaining diet history, completing nutritional assessments, and determining appropriate care.
• Develop individualized nutrition plans that are sensitive to the cultural and economic needs of our patients.
• Provide 1:1 nutrition counseling on healthy eating, chronic disease prevention and dietary issues.
• Monitor, evaluate and document outcomes of nutrition assessments, plans and progress.
• Maintain records of nutritional care and document notes on all patient visits in EHR system.
• Collaborate to create culturally sensitive educational material about healthy eating habits and chronic disease prevention.
• Facilitate group education sessions on nutrition topics relevant to our patients.
• Actively participate in departmental and/or interdisciplinary meetings, task forces, and projects such as quality improvement initiatives.
• Collaborate with Community Health and other departments to identify and obtain dietary and nutrition resources for patients.
• Serve as a resource person to other members of the CRCH team around the dietary needs of patients and provide support to other members of the clinical and administrative team in an accessible, open manner.
• Communicate effectively and clearly with members of the care team regarding patient status and needs.
• Work collaboratively with other Health Center staff in meeting JCAHO and other regulatory and/or funding requirements.
• Perform other nutrition-related duties as required by the Medical Director or designee.
