Citadel Care Centers is seeking a qualified Dietitian to assess residents' nutritional needs, develop and implement care plans, and provide counseling as needed. You will conduct initial, quarterly and annual reviews in line with state and federal guidelines and collaborate with physicians and staff to optimize dietary care.

Qualifikationen

• Bachelor's degree in Nutrition and Dietetics.
• State Licensure or Certification as a Registered Dietitian by state law.
• 900 hours of supervised practice under the supervision of a registered dietitian or nutrition professional.
• Computer literacy: Outlook, Office, Internet, EMR charting.
• At least two years of experience in related dietary field (preferred).
• Ability to read, write, and speak English; communicate effectively verbally and in writing.

• Complete initial, quarterly and annual nutritional reviews on residents.
• Review weights to assess changes and adjust care plans as needed.
• Attend care plan conferences, QA meetings, and clinical meetings to share information.
• Counsel residents, families, and staff on dietary requirements and resolve issues.
• Consult with physicians and other health care staff to assess dietary restrictions and needs.
