Job summary

Legacy Health in Portland, OR seeks a Clinical Dietitian to provide evidence-based nutrition care across inpatient and outpatient settings. You will perform nutrition assessments, create individualized care plans, and deliver medical nutrition therapy for diverse conditions in collaboration with physicians, nurses, and allied health teams.

You will document in the EMR, educate patients and families, and contribute to quality/safety initiatives and community health, aligned with Legacy’s

Qualifications

• Registered Dietitian Nutritionist (RDN) required or eligible.
• CDR credential required.
• Oregon licensure as a Dietitian/Nutritionist or eligible.
• Completion of ACEND-accredited program with supervised practice.
• CNSC credential preferred.
• Experience with EMR documentation.

Responsibilities

• Perform comprehensive nutrition assessments for diverse patient populations.
• Develop and implement individualized nutrition care plans and interventions.
• Provide medical nutrition therapy for acute and chronic conditions.
• Collaborate with physicians, nurses, and interdisciplinary teams on treatment plans.
• Document assessments, plans, and progress in the EMR.
• Educate patients, families, and caregivers on nutrition and lifestyle changes.
• Participate in quality improvement and patient safety initiatives.
• Support community health and preventive care programs.
