The Clinical Dietitian at ICCS is a key member of the interdisciplinary team, responsible for nutrition care, disease management, and education of patients with chronic conditions. The role involves developing and implementing cost-effective, patient-centered care plans aimed at optimizing clinical and fiscal outcomes, emphasizing health promotion and disease prevention. The Dietitian manages the continuum of care with evidence-based strategies to support self-managed disease.
