Summary:

  • Responsible for the clinical nutrition care of patients at the Diabetes Education Center at Pennsylvania Hospital. Functions as a professional consultant and member of the healthcare team providing high-quality, evidence-based nutrition care for patients in the ambulatory care setting. Acts as a patient educator, performing nutrition assessments, providing nutrition education, and developing nutrition care plans. Actively manages nutrition therapy interventions including care of patients with prediabetes, type 2 diabetes, type 1 diabetes, latent autoimmune diabetes of adults, and patients referred for medical nutrition therapy for other diagnoses as applicable. Communicates with multidisciplinary care team as appropriate. Participates in department team-based education programs and quality assurance initiatives. Maintains comprehensive medical nutrition therapy knowledge and engages in continuing education. Provides nutrition care as part of an interprofessional team, respecting individual dignity and autonomy.

Responsibilities:

  • Direct Patient Care: Perform nutrition and diabetes assessments, develop intervention plans, and monitor outcomes. Demonstrate competence with diabetes technology such as blood glucose meters, CGMs, smart insulin pens, and insulin pumps. Collaborate with specialists to review and provide patient recommendations based on downloaded reports. Understand and teach about diabetes and weight management medications. Apply evidence-based practice guidelines. Communicate effectively with the healthcare team. Provide patient-centered care with respect and sensitivity. Tailor nutrition assessment and care to different age groups.
  • Professional Performance: Provide quality services ethically and responsibly. Ensure safe, evidence-based practice. Use research to inform care. Communicate effectively. Manage resources efficiently. Demonstrate professionalism and teamwork. Maintain confidentiality. Balance patient care with organizational priorities.
  • Clinical Activities: Provide patient education and participate in support groups and classes. Engage in projects and quality initiatives. Develop educational materials. Participate in community outreach.
  • Education: Maintain registration and licensure through continuing education. Serve as a preceptor and support student shadowing.

Credentials:

  • Registered Dietitian (Required)
  • Licensed Dietitian-Nutritionist (Required)
  • State of Pennsylvania Licensed Dietitian Nutritionist (LDN) within 6 months of hire.

Education or Equivalent Experience:

  • Bachelor of Arts or Science (Required)
  • Completion of an ACEND-accredited dietetics program or equivalent supervised practice training. 0-1 years related experience preferred.
