Job summary

Miami Jewish Health is seeking a Clinical Dietitian to provide nutrition care for Florida PACE Centers participants across care settings. The role requires evaluating nutritional needs, educating patients and caregivers, and supporting care planning within a multidisciplinary team.

Qualifications

  • BS or advanced degree in food and nutrition or dietetics.
  • One year experience working with the frail elderly population.
  • Current Florida Dietitian/Nutritionist license; ADA registered or Registered Dietitian eligible.

Responsibilities

  • Provide nutrition education to participants and caregivers as ordered by physician or requested by nursing or other disciplines.
  • Perform nutritional assessments and periodic reassessments as indicated by need and FPC standards.
  • Participate in care planning for initiation, progression, and maintenance of participants receiving tube feedings, TPN and PPN.
  • Attend multi-disciplinary team conferences for FPC participants.
  • Provide nutrition education to staff and community as indicated.
  • Work collaboratively with other care disciplines in development of the team plan of care.

Job Summary

Provides nutrition care to Florida PACE Centers participants in all care settings.

Essential Job Functions
  • Responsible for providing nutrition education to participants and caregivers as ordered by physician or requested by nursing or other disciplines

Experience

  • Requires ability to teach, analytical thinking and ability to provide care to geriatric participants
