Job summary

CHI Living Communities is seeking a compassionate Clinical Dietitian to promote health and quality of life for residents on a 240-acre campus in Centerville, Ohio. The role focuses on nutrition care across independent living, assisted living, memory care, and skilled nursing facilities.

You will complete assessments, develop care plans, monitor at-risk residents, and collaborate with the care team to ensure high standards of dietary services and regulatory compliance.

Qualifications

• Bachelor's degree in Dietetics from an accredited university.
• Long-term care experience preferred.
• Strong communication and organizational skills.
• Compassionate approach with a commitment to quality resident care.

Responsibilities

• Complete nutritional assessments and develop appropriate care plans.
• Monitor residents with weight loss and other high nutritional risks.
• Provide nutritional support for residents with specialized dietary needs.
• Review MDS assessments, weights, and care plans.
• Collaborate with nursing, dietary, and other members of the care team.
• Provide education to dietary and nursing staff.
• Monitor food safety, sanitation, menus, and dining services.
• Participate in quality improvement and regulatory compliance activities.

Our beautiful 240 acre campus is one of the largest Continuing Care Retirement Communities in the United States. St. Leonard features independent living, assisted living, memory care, skilled nursing, rehabilitation services, and adult day care as well as a community wellness center that encompasses a fitness center, and restaurant.

St. Leonard is seeking a compassionate and knowledgeable Clinical Dietitian to join our team. This position plays an important role in promoting the health, nutrition, and quality of life of our residents.

Job Responsibilities

• Complete nutritional assessments and develop appropriate care plans.
