Qualifications

• Master's degree in Clinical Counseling is required.
• 5+ years of work experience in a related job discipline.
• Must have an Indiana license or the ability to obtain a license in Indiana before credentialing date; SW III or CC III - LMHC in Indiana.

Responsibilities

• Psychosocial Assessment: perform comprehensive assessments of complex social situations using advanced interviewing skills and experience to identify challenges and inform interventions.
• Resource Management: develop and manage resource networks, assist families to obtain financial aid and community resources, and advocate for patient populations.
• Psychosocial Interventions: provide counseling and education on treatment options, support self-management for high-risk patients, and coordinate with the care team to optimize outcomes.
• Interdisciplinary Collaboration: coordinate with hospital teams and outside agencies to minimize fragmentation and promote patient well-being.
• Collaboration: facilitate safe care transitions, advocate at system level, and foster professional growth within the team.
