Net-Centers-Net-Community-Care in Bridge - Philadelphia, PA 19137 seeks a Counselor to provide assessment and treatment services and maintain an active caseload. Client charts must meet Pennsylvania Department of Health, city, county, federal, and CARF standards as well as payor requirements.

The role requires ongoing training, licensure maintenance, and strict adherence to applicable laws and CARF standards. Urine drug screens and coordination of care are required.

Qualifications

• Annual 25 hours of training; half internal, half external.
• 10 hours of HIV, STDs, and TB training.
• Maintain licensure and certification as required.
• CCDP PCB certification strongly recommended.
• Adhere to laws, CARF standards, and payor contracts.

Responsabilités

• Provide assessment services using standard instruments to determine level of care (LOC).
• Maintain an active client census with sessions at least twice monthly, weekly when appropriate.
• Maintain clinical charts: Comprehensive Recovery Plan, progress notes, updates and reviews.
• Coordinate psychiatric referrals and aftercare planning; ensure documentation aligns with DOH, CARF standards.
• Timely documentation and discharge planning; perform urine drug screens.
• Other duties as assigned.
