Job summary

Odyssey House is seeking an Outpatient Counselor for the Bronx Outpatient Program. You will provide counseling, case management, and discharge planning to clients with substance use and co-occurring issues.

Work with a clinical team in a recovery-focused setting, coordinating care, maintaining records, and participating in ongoing training and quality improvement activities. The role requires flexible scheduling and bilingual Spanish is a plus.

Qualifications

  • CASAC or CASAC-T preferred or ability to obtain within 6 months of hire.
  • Ability to deliver culturally competent services to a diverse client population in an urban setting.
  • Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook).
  • Must be able to work a flexible schedule.
  • Bilingual Spanish preferred

Responsibilities

  • Administer comprehensive psychosocial assessment tools at intake and at various points in accordance with Odyssey House policy and procedures.
  • Provide individual counseling, group counselling and case management onsite and in the home or community.
  • Formulate and monitor a comprehensive individualized treatment plan with objectives and updates as needed.
  • Maintain up-to-date client clinical records including treatment planning, notes, and social determinants of health.
  • Provide crisis intervention services.
  • Responsible for ongoing discharge planning beginning at admission.
  • Assess changes in clinical needs and adjust care accordingly.
  • Coordinate effective care with collateral services, families, and criminal/family justice agencies.
  • Deliver services in line with evidence-based curricula and training.
  • Liaise with referral agencies and provide regular progress reports.
  • Build community and agency affiliations to meet client needs.
  • Educate and support family members regarding client issues.
  • Collect specimens for toxicology testing as required.
  • Attend staff meetings, case conferences, and trainings.
  • Participate in quality improvement activities.
  • Perform other duties as required.

Major Functions

The Outpatient Counselor is responsible for providing counseling and case management services related to substance abuse, legal, mental health, medical and vocational/educational issues. The Outpatient Counselor documents all provided services in a manner that is compliant with regulatory and third-party payor requirements. She/he works to coordinate and provide treatment services based on the individualized treatment plan including discharge planning. The Outpatient Counselor advocates for and promotes a recovery-based lifestyle. The Outpatient Counselor works closely with the rest of the staff as an integral part of the clinical team.

Specific Duties & Responsibilities
  • Administer comprehensive psychosocial assessment tools at intake and at various points in accordance with Odyssey House policy and procedures.
  • Provide individual counseling, group counselling and case management services onsite and in the home or community.
  • In collaboration with each client, formulates a comprehensive individualized treatment plan, monitors achievement of objectives, reviews and updates the treatment plan in accordance with clinical needs of the client.
  • Maintain up-to-date client clinical records including at minimum: treatment planning, group counseling notes, progress notes, and the monitoring of social determinants of health in accordance with regulatory guidelines and program standards.
  • Provide crisis intervention services.
  • Responsible for ongoing client discharge planning beginning at admission.
  • Provides ongoing assessment of each assigned client, monitoring for any changes in clinical needs.
  • Assures timely an effective care coordination with collateral services providers, family members and criminal/family justice agencies.
  • Delivers clinical services in accordance with evidence-based curricula and training.
  • Acts as a liaison with all referral agencies providing regular verbal and written progress reports.
  • Establish community and agency affiliations which address the client’s need and strengthen the delivery of services.
  • Provide education and direct services to family members, as appropriate, regarding client’s substance use/misuse and related behavioral issues.
  • Conduct collection of samples for toxicology randomly and as needed.
  • Attend regularly scheduled staff meetings and case conferences.
  • Attend all required in-service training seminars.
  • Participate in quality improvement activities.
  • Other relevant duties as required.
REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES
  • CASAC or CASAC-T preferred, or the ability to obtain within 6 months of hire.
