About Us

Breaking Ground operates nearly 4,000 units of housing across New York City, as well as in upstate New York and Connecticut. We focus on supportive housing—affordable housing paired with services to help people maintain their homes long-term. Our programs help those experiencing street homelessness, especially the longest-term residents, to come indoors through outreach and transitional housing, guiding them towards permanent housing.

Position Overview

The Clinical Coordinator, reporting to the Assistant Director of Programs, supervises Case Managers and Consumer Advocates for the Safe Options Support (SOS) team serving The Rockaways. They manage a small caseload and ensure the use of Harm Reduction and Housing First strategies, along with other clinical interventions to provide high-quality client care. The Clinical Coordinator also maintains client records in compliance with regulatory and agency standards.

Work Schedule

Primarily Monday through Friday during business hours, with off-hours availability for emergencies or special projects.

Essential Duties and Responsibilities
  • Supervise a team of Case Managers and Consumer Advocates
  • Conduct regular supervision meetings
  • Implement case management strategies based on Housing First, Harm Reduction, CPI, Motivational Interviewing, and trauma-informed care models
  • Develop and oversee individual service plans; ensure thorough documentation
  • Assess potential clients
  • Provide clinical direction, troubleshoot, and strategize around client engagement and behaviors
  • Promote a culture emphasizing housing placement readiness and success
  • Evaluate client functioning and supervise case conferences
  • Coordinate care with outreach teams and housing providers
  • Support staff in managing difficult situations and de-escalation
  • Perform additional duties as assigned
Minimum Qualifications
  • NYS Licensed Masters Social Worker (LMSW); must obtain LMSW within six months of employment
  • At least 3 years of experience with related populations; supervisory experience preferred
  • Strong clinical understanding of Housing First, Motivational Interviewing, Harm Reduction, and health systems
  • Experience with crisis intervention and risk assessments
  • Ability to work effectively with diverse stakeholders
  • Proven success in fast-paced environments with multiple priorities
  • Leadership skills to motivate staff and achieve goals
  • Excellent communication skills, both verbal and written
  • Valid driver’s license for NY, NJ, CT, or PA with a good driving record
  • Proficiency in Microsoft Office Suite; familiarity with databases and quick learner of new programs
