Job summary

MidState Medical Center, part of Hartford HealthCare, is seeking a Clinical Coordinator (RN) to support the Outpatient Spine and Pain Center. You will assess patient conditions, set goals, determine nursing actions, and coordinate discharge planning.

You will triage in a fast-paced environment, assist physicians, maintain standards of care, and collaborate with therapy teams to ensure seamless nursing care through procedures.

Qualifications

• ASN required and CT RN license are essential.
• CPR and ACLS certification are required.
• Strong written and verbal communication skills are essential.

Responsibilities

• Assess patient conditions and set goals for care.
• Prescribe nursing actions to meet patient needs including discharge planning.
• Coordinate nursing care with therapy and other departments.
• Maintain hospital nursing standards and follow policies and procedures.
• Prioritize tasks and triage in a fast-paced environment; assist physicians as needed.

Maintains the Outpatient Spine and Pain center position. The Clinical Coordinator (RN) assesses the patient's condition, sets goals, prescribes appropriate nursing actions to meet those goals and the physical, psychological, social and rehabilitative needs of the patient, including discharge planning. Assist in the coordinating nursing care of patients when scheduled for therapy or procedures by other departments. Establishes and maintains communications and utilizes teaching opportunities with patients, and other significant individuals. Maintains the standards of nursing care and implements the policies and procedures of the hospital. Must be able to prioritize and work in a fast paced environment to triage, and assist physicians as needed.

Valid CT RN license,

C.P.R. & ACLS certification required with excellent written verbal and communication skills.
