Job summary

Kalamazoo Family Health Center Inc. is seeking a Clinical Coordinator – MA or LPN to support patient wellbeing under medical director guidance. This full-time role in Kalamazoo, MI involves coordinating communications, scheduling, and documentation to ensure timely follow-up and safe patient care.

You will act as liaison among patients and care teams, manage lab results, refills, and education, and uphold privacy and compliance while participating in continuous improvement activities.

Qualifications

  • One or more year of related experience and/or training preferred, or equivalent combination of education and experience.
  • Bilingual (English and Spanish) desired.
  • CMA, RMA, or LPN credentials preferred.
  • Must navigate EHRs and be proficient with medical terminology.

Responsibilities

  • Collaborate with internal and external care team members to promote patient safety and optimize health outcomes.
  • Prioritize tasks based on patients' immediate needs and organizational priorities.
  • Answer patient phone calls, gather information, and orchestrate timely follow up on a wide variety of patient needs.
  • Support patient care team by maintaining accurate patient records, sending/receiving faxes, and following up on medication refill requests.
  • Notify patients of lab results as indicated and under provider direction.
  • Document clinical actions in the EHR and organize documentation so that patient records are accessible.
  • Utilize the EHR to schedule appointments, communicate with care team, and provide education/recommendations as outlined by a provider.
  • Support and adhere to administrative and clinical policies and participate in CQI initiatives.

POSITIONSUMMARY:

UnderthegeneraldirectionoftheMedicalDirector,theClinicalCoordinatorsupportsthewellbeingofallFamily HealthCenterpatientsbyensuring timelycommunication,action,andfollow upona widevarietyof patient relatedconcerns.TheClinicalCoordinatorfunctionsasaliaisonbetweenpatientsandmembersofthecare teamthroughcommunication(viaphone,fax,electronichealthrecord,in-person,orotherwise),carryingout providerinstructions(i.e.,communicating labresults,refillingprescriptions,etc.),andmaintainingmeticulous documentation.

DUTIESAND RESPONSIBILITIES:

  • One or more year(s) of related experience and/or training preferred, or equivalent combination ofeducationand experience.
  • Bilingual (English and Spanish) desired.
  • CMA, RMA, or LPN credentials preferred.
