Qualifications

• Graduate of an ARC-PA accredited or NYSED approved PA program.
• Minimum of 5 years related experience.
• NYS PA License, NCCPA Board Certification, BLS, NYS DEA.

Responsibilities

• Coordinate surgical and medical management of kidney and pancreas transplant patients in outpatient settings.
• Manage living donor follow up with living donor coordinators.
• Coordinate care for surgical patients (hernia repairs, dialysis catheters, etc.).
• Assist transplant surgeons and nephrologists with history, exams, planning, and documentation.
• Conduct pre-operative history and physicals and obtain transplant consents.
• Provide direct patient care for awaiting transplant, post-transplant, and donors.
• Collect, order, and interpret lab work per protocols.
• Order prior authorizations for transplant recipients.
• Develop practice protocols and patient education materials.
• Monitor patients in research studies; contribute to clinical databases.
• Participate in meetings, affiliations, and professional development.

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary

The Clinical Coordinator III- PA will function in the outpatient setting of the Kidney/Pancreas Transplant Program in collaboration with the Kidney/Pancreas Transplant multidisciplinary team to care for and manage post-transplant kidney and pancreas recipients, post-operative kidney living donors, and patients who underwent surgical or invasive procedures by the Kidney/Pancreas transplant surgeons. The position will hold a combination of 3-5 half day outpatient office or telehealth sessions per week, with patient care provided at the CUIMC main campus as well as outreach locations across the tri-state area.

Responsibilities

• Coordinate surgical and medical management of kidney and pancreas transplant patients in the outpatient setting.
• Manage living donor follow up in collaboration with living donor coordinators.
• Coordinate care of surgical patients that undergo hernia repairs, dialysis catheters, and other elective surgical procedures conducted by our kidney and pancreas surgeons.
• Assisting transplant surgeons and nephrologists in the outpatient clinic to perform comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient’s response to care.
• Conduct pre-operative history and physical exams, as well as obtain transplant and other surgical consents.
• Provide direct patient care by evaluating patients awaiting transplant, as well as post- transplant recipients, and kidney donors.
• Collect, order and interpret lab work and diagnostic studies under agreed upon protocols.
• Order and obtain medication prior authorizations for kidney transplant recipients.
• Develop practice protocols, standard of care, and patient education material for kidney/pancreas transplant recipients.
• In conjunction with the research team, monitor and provide care to patients participating in research studies.
• In partnership with Quality, Research and Education group, help develop and manage detail clinical databases to evaluate and improve patient outcomes.
• Attend professional meetings and conferences, maintain professional affiliations and enhances professional development to stay current with trends in health care administration.

Minimum Qualifications

Minimum education:

• Graduate of an ARC- PA accredited or New York State Education Department approved PA education program

Minimum related experience:

• 5 years of related experience

Minimum licensing / certifications required upon start date:

• NYS PA License and NCCPA Board Certification.
• BLS Certification
• NYS DEA Certification

Other Requirements

• Proficiency with technology and social media platforms.
• Effective interpersonal and communication skills required.
• Intermediate to advanced level MS Office skills.
• Work requires manual dexterity for handling charts and using office equipment - specifically the use of a computer keyboard and calculator. Must be able to stoop and bend to retrieve files and supplies, and sit, stand or move around for extended periods of time.
