Clinical Coordinator - Facilities MA, LPN, RN

We are seeking a Clinical Coordinator to assist with all clinical needs for our Conway facility patients.

The Clinical Coordinator plays a crucial role in ensuring the seamless coordination of patient care. This position involves proficient chart preparation, communication, scheduling of patient visits, appropriate utilization and logging of chronic care management activities, and provider and patient support.

This will be a traveling role, covering ALF, SNF, and IL facilities in the Conway area. You must have reliable transportation as travel is required daily. This is a full-time, salaried position (8:00 AM - 5:00 PM, Monday-Friday).
