Job summary

Palm Medical Centers is seeking an experienced Clinical Coordinator to lead care planning for hospital and nursing home patients, from admission through discharge, ensuring seamless continuity of care.

The role requires strong knowledge of case management, reimbursement, and collaborative teamwork, with emphasis on coordination of services, crises intervention, and effective communication with care teams and families.

Qualifications

• Proven experience in case management or mental health care management.
• Excellent knowledge of case management principles and reimbursement.
• Experience with psychological aspects of care is preferred.
• Strong communication and organizational skills.
• Ability to multi-task and work as part of a team.
• Bilingual preferred; familiarity with CPT/ICD-10 and guidelines.

Responsibilities

• Admit new clients by reviewing records and orientations.
• Track hospital and nursing home admissions and discharges for stats and costs.
• Determine patient needs via intake interviews and evaluations.
• Coordinate treatment programs and arrange resources including transport.
• Monitor progress, advocate for services, and intervene in crises when needed.
• Maintain and review patient records and progress notes.
• Communicate with managers about discharges and post-care follow-ups.
• Lead weekly interdisciplinary meetings to discuss client progress.
• Improve processes and outcomes by reviewing practices and policies.
