We’re Hiring a Clinical Coordinator to Join Our Team!

This full-time position is impactful and directly influences the lives of underserved, vulnerable, and at-risk populations. We seek candidates who leverage their clinical knowledge and critical thinking skills to triage effectively, delegate tasks, identify risks, and close gaps in care. Responsibilities include contacting individuals with out-of-range vital signs or device readings, assessing immediate health needs, reminding them of available resources, and documenting interactions. As a Clinical Member Coordinator, you will receive real-time data through remote patient monitoring devices and be supported by our online platform, ensuring high adherence to care plans. This role is fully remote but may require travel to our offices in Columbia, MD, up to four times per year for company events and meetings.

Key Responsibilities

  • Provide telephonic consultation to triage and assess patient cohorts for disposition.
  • Re-engage non-compliant members through focused outreach.
  • Collaborate with clinical services, health coaching, and member support teams to coordinate care.
  • Develop, communicate, and maintain strong working relationships within the clinical team.
  • Facilitate and delegate patient education using appropriate modalities.
  • Assist members with mobile care management devices and maximize resource access.

Required Qualifications

  • Current and active medical certification (e.g., MA, CNA, MedTech, LPN, Health Coach, Case Management).
  • 1-3 years of experience in a medical care setting.
  • Knowledge of chronic disease management (e.g., Diabetes, Hypertension, Heart Failure, Asthma, COPD, Weight Management, Behavioral Health).
  • Proficiency with technology and web applications.
  • Ability to work effectively remotely with strong communication and collaboration skills.
  • Effective communication skills with compassion and empathy.
  • Exceptional interpersonal, organizational, and communication skills.

Additional Preferred Skills

  • Experience in urgent/emergent or front/back medical office care settings.
  • Experience working with diverse populations.
  • Understanding of Medicaid and Medicare.
  • Experience with telephonic outreach to providers or patients.
  • Knowledge of HEDIS and NCQA standards.
  • Fluency in Spanish is preferred but not required.

Location Preference: While this role is remote, candidates in Florida, Illinois, and Kentucky will receive priority consideration.
