Job summary

BrightStar Care in Novi, MI is seeking a Registered Nurse to join our team as a Full Time RN Clinical Case Manager. You will provide high-quality home health nursing care, perform initial assessments, develop and update plans of care, and supervise staff to ensure compliant service delivery.

The role emphasizes compassionate patient care, HIPAA-compliant documentation, and collaboration with families and caregivers.

Qualifications

• Current unrestricted RN license in good standing.
• Ability to supervise and coordinate care for home health clients.
• Strong nursing assessment and planning skills.

Responsibilities

• Provides care within the scope of practice and reevaluates client nursing needs.
• Performs initial assessments and plan of care, with regular reassessments.
• Conducts supervisory visits and contributes to staff training.
• Performs skilled nursing tasks such as injections, wound care, and medication setup.

Responsibilities

• Provides care as defined within the scope of practice
• Makes the initial evaluation visit and regularly reevaluates the home care client’s nursing needs
• Prepares the clients initial assessments and plan of care
• Completes regular supervisory visits and reassessments on clients and care providers
• Assists with in‑service and training programs
• Performs skilled services such as injections, wound care, medication set up, cath care, etc.

Requirements

• Current unrestricted RN license
• Reliable car/auto insurance
• Clean background and criminal record
• Understands regulatory compliance with state and federal regulations
• Adhere to HIPAA and maintain client confidentiality
• Demonstrated ability to supervise and direct staff.
• Possess exceptional comprehensive nursing assessment skills
• Comply with accepted professional standards of practice.
• Treat clients, staff and the public with courtesy, respect and presents a positive public image.
