Job summary

Lutheran Social Services of South Dakota is seeking a dedicated Case Manager for the New Alternatives program to support youth as they move into adulthood. You will develop individualized case plans, perform ongoing assessments, coordinate medical and behavioral health services, and connect young people with community resources.

This full-time role requires clear communication, strong organizational skills, and a compassionate approach to families.

Qualifications

• Bachelor’s degree in social work or related field required.
• Must be at least 21 years old.
• Licensure preferred: LSW or LSW-A in SD.
• Willingness to undergo background checks.
• Valid auto-insurance and driver's license or ability to obtain one.

Responsibilities

• Develop individualized case plans and conduct ongoing assessments.
• Coordinate medical, dental, and vision services for youth.
• Maintain accurate case documentation and support families.
• Build partnerships with community providers and service partners.
• Transport clients to appointments as needed and review MARs for accuracy.

RESPONSIBILITIES:

As part of the LSS team, the Case Manager helps develop individualized case plans, conducts ongoing assessments, and connects youth with appropriate community resources. This position is essential in identifying the needs of youth and their families, supporting their growth, and fostering positive change in their lives. Success in this role requires strong communication and organizational skills, along with a deep commitment to helping others. Responsibilities include maintaining accurate case documentation; building collaborative relationships with community partners and service providers; coordinating medical, dental, and vision appointments; ensuring annual exams are completed; supporting medication management processes in collaboration with nursing staff and pharmacy providers; obtaining medical records and treatment consents; reviewing Medication Administration Records (MARs) for accuracy; and transporting clients to appointments as needed.

QUALIFICATIONS:

• A bachelor’s degree in social work or a related field, is required.
• Must be at least 21 years of age, is required.
• Licensed Social Worker or Licensed Social Work Associate through the South Dakota Board of Social Work Examiners is preferred.
• Experience with client Medication Administration Records (MARs), is preferred.
• Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
• Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
